COMPLEX TREATMENT OF PATIENTS WITH PHLEGMONAS OF THE MAXILLO-FACIAL AREA AND NECK IN COMBINATION WITH LYMPHOTROPIC ANTIBACTERIAL THERAPY, VACUUM DRAINAGE AND PLASMOLIFTING

Relevance. The lymphatic system plays an important role in the development and pathogenesis of purulent-inflammatory diseas-es. It provides a delay in the microorganisms in the lymph nodes where the so-called "toxic depot" is created. Thus, endolymphatic administration of antibiotics, immunomodulators, antioxidants, anticoagulants is pathogeni-cally substantiated. The second very important method of treat-ment of phlegmon at the stage of reparation is plasmolifting. This is one of the well-known means of biomessotherapy, which uses the patient's own biomaterials, which provides complete biocompatibility and eliminates the possibility of allergic reac-tions.
The purpose of the experiment. To increase the effectiveness of the antibiotic on the inflammatory process in the exudative phase, and to stimulate the reparative processes in the wound at the proliferation stage during the treatment of patients with phlegmon of the maxillofacial area.
Materials and methods. 120 patients with odontogenic phlegmon were treated. At admission to the hospital all patients were divided into 2 groups: 1st group – 60 patients whose treat-ment was carried out by the traditional method. 2nd group – 60 patients, the treatment of which consisted in the fact that the lymphotropic injection of the second antibacterial drug was added to the traditional method of treatment. And autoplasma enriched with platelets was used at the reparation stage.
Conclusions. Regional lymphotropic injection of the antibiotic in the treatment of phlegmon of the maxillofacial area and neck leads to a more effective impact on the inflammatory process, accelerates the transition to the second phase of inflammation. The injection of platelet-rich autoplasma into the soft tissues around the inflammatory focus stimulates proliferation and ac-celerates the process of regeneration and wound healing.
