MANAGEMENT OF CHILDREN WITH CONGENITAL UNILATERAL CLEFT LIP AND PALATE USING A MULTIDISCIPLINARY APPROACH

The high prevalence of congenital cleft lip and palate (CLP), the scope and duration of rehabilitation require optimization and justification of the timing and sequence of interventions within the framework of multidisciplinary measures. Purpose: substantiation of medical and diagnostic measures determined in time, terms, sequence and volume based on etiopathogenetic relationships of dentognathic deformities and congenital unilateral cleft lip and palate lip, alveolar process, hard and soft palate using a multidisciplinary approach. Materials and methods. The complex of step-by-step treatment and diagnostic measures for children with congenital unilateral cleft lip and palate (UCLP) was developed and tested on 137 patients who were in the period of senescence temporary occlusion and the first period of permanent occlusion. Results and Discussion. In the management of children with UCLP, a set of measures proposed, which carried in three periods: pre-surgical, the period of primary surgical interventions, and after primary surgical interventions. Morphological, anamnestic-medical, dental and psychological aspects were taken into account in each period. Conclusions. Studies before, during the treatment and during the rehabilitation period of children with congenital UCLP have demonstrated in many cases the absence of important components of the management of this category of patients. A complex of measures in the pre-surgical, the period of primary surgical interventions, and after primary surgical interventions, which take into account morphological, anamnestic-medical, dental and psychological aspects, make it possible not only to create individualized management programs, but also to monitor their effectiveness using clinical and paraclinical examination methods, morphometry, anthropometry, computer tomography, speech therapy and psychological tests. This approach is significantly increase the effectiveness of treatment and rehabilitation of this contingent of children, as evidenced by the results of our research.
